Basic informations
Please attach a short CV of the author(s) written in third person, including a selected film list and a list of awards granted (if applicable). (max. 2000 characters with spaces).

Preliminary budget for the film—such budget should contain combined introductory cost of the project (including documentation, copy rights and licences, travels, prospective administrative costs, taxes and other public obligations, etc.), estimated cost of production (including remuneration of the film crew and authors, stage design, cost of equipment, film plan support etc.) and estimated cost of post-production (including cost of editing, sound engineering etc.). Budget should contain the total cost of production of a film entering the competition. The competition jury will revise the accuracy of the budget preparations, and budget adequacy to the film production and artistic premises. Final budget for the film will be submitted by the Laureate no later than 5 days prior to signing the Agreement with the Museum, with the stipulation that the difference between the final budget and the preliminary budget cannot exceed 10%, nor can it exceed the sum of 56,000 PLN gross.

In response to the call for entries to the competition I thereby propose to produce one film in consonance with the competition offer submitted and meeting the competition requirements for the gross sum of: _______________________ (the sum offered cannot exceed the amount of 56,000 PLN (ca 15 000 USD) gross).
I have read and accepted the documentation pertaining to the competition, including: formal call for competition entries, regulations (appendix no. 1 to the procurement offer) and the Contract (Relevant Provisions for the Parties entering into Contract).
I hereby agree that my personal data included in this form will be processed in order to proceed with the competition procedure, in accordance with the Act of 10 May 2018 on the Protection of Personal dara (Journal of Law, 2018, item 1000), and the regulations of the European Parliament and the Council from 27 April 2016 pertaining to the Article 5 of Regulation (EU) No 182/2011 of the European Parliament and of the Council of 16 February 2011 laying down the rules on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and the avoidance of the directive 95/46/WE (“GDPR”).

Your personal data will be processed for the purpose of conducting the statutory activity, mission and tasks of POLIN Museum pursuant to Article 6(a) of the Regulation, i.e. the data subject’s consent, Article 6(b), i.e. performance of agreements, and Article 6(c), i.e. compliance with a legal obligation to which the controller is subject. Your personal data may be provided to the following recipients: suppliers of IT systems and IT services; entities that provide the Museum with accounting services, service quality studies, recovery of claims, as well as legal, analytic, and marketing services; postal operators and couriers; operators of electronic payment systems and banks, in the scope of execution of payments; bodies authorized by law to receive your personal data. Your personal data will not be transferred to any third country. Failure to provide the personal data required by the Museum will result in the inability to provide services and perform agreements.
The gross amount that you provide in the competition entry offer includes all the costs incurred by the competition Organizer, including tax or fiscal charges that may be imposed in the process.
I shall implement the subject of the procurement in consonance with the requirements listed in the procedure documentation.
In case the Organizer shall select my competition entry offer I hereby commit myself to enter into contract on conditions listed in the “Contract” documentation (Relevant Provisions for the Parties entering into Contract).
